 Problems in the nomenclature, definition and classification of ischemic bowel lesion has become clear as follows. 1) Ischemic bowel lesion is a multifactorial disorder and is not caused only by blood perfusion disturbance. 2) The transient type and the stricturing type in the Marston's classification are not independent of each other, a difference between them are only severity of the disease. The etiological classification should be established. 3) Ischemia is involved in the development and progression of various bowel disorders. In a broad sense, all bowel disorders may be accompanied by ischemic bowel lesions. 4) As the causes of ischemia have been discovered, cases of spontaneous ischemic bowel lesions in a strict sense are diminished.

 At present, the term “ischemic bowel lesion” seems to express a phenomenon rather than a disorder; from a terminological viewpoint,“ischemic bowel syndrome” may be more appropriate. When an ischemic bowel lesion is found in a known disorder, documentation of the lesion should be added to the diagnosis. A detailed description of etiology and phenomenon will contribute to established a new concept of “ischemic bowel syndrome”
