早期胃癌見つけ出し診断のためのルーチン内視鏡検査

Initial Endoscopic Examination as a Screening Procedure of Early Gastric Cancer

要旨　早期胃癌見つけ出しのための内視鏡診断能を左右するものは，①経験と読影力，①良い条件で観察する習慣，①一定の観察手順で検査すること，①異常所見を遠見と近接観察，観察方向を変えて観察する，空気量を増減して観察する，などのテクニック，①適切な器種を選択すること，①生検のテクニックとその解釈，などである．①の向上のためには所見会などで多くの症例を検討すること，①は所見会で術者の欠点を指摘し合うこと，①はルーチン検査法を確立すること，①は直視型内視鏡の欠点と利点を知ること，①は至適距離で正面から観察することが必要なこと，生検診断は観察した内視鏡診断と合わせて判断すること，などが必要であることを述べた．

　Diagnostic potency of endoscopic examination for gastric cancer depends on following factors: 1) experience and detecting ability of an operator, 2) habits of practice in good condition, 3) examination with a fixed observation order, 4) observation techniques such as using distant and near view points, changing directions of observation, changing of pneumatic expansion and etc., 5) selection of an appropriate instrument, 6) techniques of biopsy and interpretation of the biopsy specimen.

　To improve these factors, we emphasize the following requisites in each factor: 1) appraisal of many cases at case conference, 2) discussion on weak points of an operator at case conference, 3) establishment of routine examination protocols, 5) understanding of advantage and disadvantage of a forward-viewing endoscope, 6) front view of lesions at an appropriate distance is a key to make a better biopsy, and biopsy diagnosis should be made with both pathological and endoscopic findings.
