粘膜下腫瘍様の食道表在癌のX線診断

Radiological Diagnosis of the Superficial Esophageal Cancer Mimicking Submucosal Tumor

要旨　粘膜下腫瘍様所見を示した食道癌8例〔粘膜下層（以下sm）癌5例，進行癌3例〕について検討した．組織型をみると，中分化型扁平上皮癌2例，低分化型扁平上皮癌3例，類基底細胞癌1例，粘表皮癌1例，腺様囊胞癌1例であった．組織型と隆起の性状をみると，類基底細胞癌は多結節の隆起としてみられた．隆起の表面に陥凹がみられるものは低分化扁平上皮癌でlymphoid stromaを伴う場合と腺様囊胞癌などの場合があった．X線検査による存在診断はsm以深の隆起型癌なのであまり問題はなかった．

　The eight cases of esophageal cancer mimicking submucosal tumor (5 cases of submucosal cancer and 3 cases of advanced cancer) were evaluated. Histologically, there were 2 cases of moderately differentiaed squamous cell carcinoma, 3 cases of poorly differentiated squamous cell carcinoma, 1 case of basaloid carcinoma, 1 case of mucoepidermoid carcinoma and 1 case of adenoid cystic carcinoma. From the viewpoints of relationship between histological type and shape of protrusion, the basaloid carcinoma was detected as a protruding lesion with multiple nodules. The protruding lesions with depression on the surface may be the poorly differentiated squamous cell carcinoma with lymphoid stroma or the adenoid cystic carcinoma. There was almost no difficulties in detecting the lesions by the radiological examination, because they were protruding lesions with sm or deeper invasion.
