 We discussed the importance and necessity of x-ray examination (double contrast) for patients with gastric cancer, especially in preoperative diagnosis. The major advantages of x-ray examination compared with endoscopic examination are as follows: 1) it shows the site and the size of tumors objectively, 2) we can obtain some information about the gastric mucosa without invading the body, and 3) we can obtain some information about submucosal involvement by taking x-ray films from different positions. Such information is very important and useful for determining the surgical procedure and the cut line in combination with endoscopical findings. The most important thing is to take x-ray films with clearity and offering as much information as possible. If it is possible, x-ray examination is important and necessary for preoperative diagnosis in combination with endoscopic examination.
