 Six of eight criteria in Western countries included ① clinical features, ② radiologic and endoscopic findings, ③ macroscopic findings of surgical specimens, and ④ pathological findings.

 When ten cases with so-called “simple ulcer” which had ileocecal ulcer were analyzed according to the Western criteria of CD, all the ten cases were diagnosed as definite CD. However, according to Japanese criteria these ten cases were not considered definite CD.

 On the other hand, CD cases with solely aphthous lesions were not diagnosed as definite CD according to Western criteria.

 Conclusion : Criteria for CD should not include clinical symptoms and should be composed mainly of characteristic findings (longitudinal ulcer, granuloma) . However, granulomatous diseases such as tuberculosis or Yersinia enterocolitis should be excluded beforehand.
