 We retrospectively examined colonoscopic diagnosis of the degree of cancer involvement in the submucosal layer in 69 lesions of sm cancer, excluding pedunculated and lateral spreading lesions. A sm cancer is categorized into sm-scanty and sm-massive cancer according to the degree of cancer involvement in the submucosal layer. From the histological cancer growth pattern in sm cancer researched by Shimoda et al, sm cancer can be divided into polypoid growth (PG) and non-polypoid growth (NPG). A deformity of the colonic arc at the site of the lesion, a granular pattern in the depressed area and a convergency of mucosal folds in colonoscopic findings are indications of sm-massive cancer. Using these findings, we were able to make a correct diagnosis in 77% of sm cancers and 78% of sm-massive cancers. The accuracy rate of our diagnosis of PG sm cancer was rather low, but, on the other land, our diagnosis was 91% correct in cases of NPG sm cancer.
