多発性偽憩室形成を伴い高度の直腸狭窄を呈した直腸粘膜脱症候群と考えられる1例

Mucosal Prolapse Syndrome with Severe Rectal Stricture, Report of a Case

要旨　患者は63歳，女性のstrainerで，約40年にわたる便秘があった．直腸指診で肛門縁から約5cmの直腸に高度の狭窄を認めた．大腸X線では下部直腸に高度の非対称性両側性狭窄像を認め，直腸前右壁側にやや幅の広い不整形の潰瘍を認めた．多点粘膜集中，偽憩室形成を伴うことから直腸の変形は多発潰瘍によるものと診断した．大腸内視鏡では肛門縁から5cmの部位に高度の全周性狭窄を認め，その肛門側には粘膜ひだの集中を伴う，比較的浅い不整形潰瘍を認めた．病理組織学的にfibromuscular obliterationの像を呈し，直腸粘膜脱症候群と診断した．高度の直腸狭窄を伴う直腸粘膜脱症候群はまれであるので報告した．

　A 63-year-old woman presented with a 40-year history of constipation requiring 15-20 minutes for every defecation. Digital examination of the rectum disclosed a severe stricture at 5 cm from the anal verge. Barium enema and endoscopy showed a severe asymmetrical stricture and multiple pseudodiverticula due to multiple ulcers in the lower third of the rectum. Pathological examination of the rectal biopsy specimen demonstrated fibromuscular obliteration leading to the diagnosis of mucosal prolapse syndrome.
