粘膜下腫瘍の形態を示した大腸癌―病理学的特徴

Histopathological Studies of Early Colonic Carcinoma Mimicking Submucosal Tumor

要旨　粘膜下腫瘍の形態を示した早期大腸癌3例を提示し，その組織発生について今まで報告された症例の検討も加えて考察した．〔症例1〕はリンパ濾胞の増生の強い腫瘍であり，この種の症例はその所見だけでも粘膜下腫瘍の肉眼所見を呈することがある．しかし，本例では囊胞状に拡張した腺管の増生，表層にみられた異型腺管は粘膜下から浸潤した組織像以外みられないこと，直腸前壁に存在したことなどから，colitis cystica profundaの癌化も否定できないと考えた．〔症例2〕は，粘膜下に膿瘍，リンパ濾胞の増生，限局性線維化など多彩な所見がみられ，それらが複合して粘膜下腫瘍様所見を呈したと考えられた．〔症例3〕は粘液癌であるが，粘液癌が粘膜下腫瘍様所見を呈するという報告はあるが，早期癌での報告はなかった．大腸癌は，胃癌や食道癌と比べて組織型が多彩でないことが，粘膜下腫瘍様の形態を示す大腸癌が少ない原因と考えられた．

　Histological evaluations of three cases of early colonic carcinoma mimicking submucosal tumor were performed. These cases are reviewed along with previously reported cases. The histological features of these cases were as follows : 1) abscess formation, 2) mucinous carcinoma, 3) carcinoma with lymphoid stroma, 4) carcinoma with massive lymphoid cell infiltration, 5) carcinoma with fibrosis around the tumor, 6) carcinoma arising from colitis cystica profunda.
