地域共生社会の実現に向けて—専門職との協働と地域包括支援センター等協議会の役割

Toward the realization of an inclusive society : Collaboration with professionals and the role of the Regional Comprehensive Support Center Council

　山形県地域包括支援センター等協議会は平成20年に任意団体として発足し，令和4年2月に一般社団法人格を取得した．現在，幅広い機関・団体と連携して，福祉の隙間を埋めるさまざまな事業に取り組んでいる．

　令和4年4月に山形県より「山形県地域包括ケア総合推進センター」を受託し，各市町村に対して「自立支援型地域ケア会議の開催支援」「地域包括支援センター職員研修会」「認知症施策の推進」「権利擁護人材育成事業」などを進めるなかで，特に「地域ケア会議の開催支援」では，言語聴覚士会をはじめ，リハビリテーション専門職と連携を図りながら高齢者等の自立支援が図られるよう，派遣調整や研修会等を開催している．

　これらの連携が高齢者等の自立支援に大きく寄与してきたが，今後，地域共生社会の実現に向けてさらなる深化が図られるよう連携のあり方を探っていきたい．

　The Yamagata Prefecture Regional Comprehensive Support Center Council was established as a voluntary organization in 2008 and was incorporated in February 2022. Currently, we are working on various projects to fill gaps in the welfare service in cooperation with a wide range of organizations. In April 2022, Yamagata Prefecture entrusted the Yamagata Prefecture Community Comprehensive Care Promotion Center to us. Since then, we have provided support to municipalities in the following areas : assistance in organizing community care conferences focused on promoting independence ; training sessions for staff at community comprehensive support centers ; promotion of dementia care policies ; programs to develop human resources for the advocacy of the human rights of people with dementia. In cooperation with rehabilitation professionals, we coordinate dispatch requests and provide training for professionals to support the independent living of the elderly. These collaborations have greatly contributed to supporting the independence of the elderly. We will explore ways to deepen cooperation further toward the realization of an inclusive society.
