Abstract Purpose:To report a case in which sinusitis was thought to have developed into the orbit via the fracture site 2 years after orbital fracture repair.

Case:A 38-year-old male patient had undergone fracture repair for a left orbital wall fracture 2 years earlier. He was referred to our clinic after a visit to his local ophthalmologist with complaints of left eye pain and periocular swelling.

Findings:CT scan showed an increased density of soft tissue in the lower left eyeball and soft tissue in the left maxillary sinus, ethmoidal sinus, and frontal sinus, which suggested the possibility that the sinus infection had spread to the orbit. On the same day, we performed a sinus release and drainage incision around the absorbable artificial bone plate and administered antibiotics, which resulted in improvement.

Conclusions:In patients with a history of orbital wall fracture, the possibility of intraorbital infection spillover after the onset of sinusitis should always be considered, and early evaluation and appropriate treatment for possible orbital cellulitis is important.
