要旨

　術前検査で穿通胎盤が疑われた37歳の女性に対し，resuscitative endovascular balloon occlusion of the aorta（REBOA）を用いて二期的子宮摘出術（delayed hysterectomy）を行った。出血量は約9,000mlと少なくなかったが，REBOAを用いることで比較的安全に術中管理を行えた。

　Placenta percreta is the most severe form on the spectrum of placenta accreta and is a cause of maternal death at delivery. A 37-year-old patient（5-parity）suspected of having placenta percreta at the preoperative examination underwent a delayed hysterectomy, and the hysterectomy was safely managed with resuscitative endovascular balloon occlusion of the aorta（REBOA）. The use of REBOA in delayed hysterectomy for placenta percreta may reduce the amount of blood loss.
