I．はじめに

　骨巨細胞腫は長管骨骨端部に好発する骨腫瘍であり，椎体に発生することは比較的稀である．本腫瘍は放射線治療により悪性転化することがあり，外科的治療が第一選択と思われる．

　今回われわれは，第4胸椎椎体に発生し硬膜外より脊髄を圧迫し対麻痺を星し，Anterolateral Approachにより全摘し得た骨巨細胞腫の1症例を経験したので報告する．

A case of giant cell tumor involving the Th4 verte-bral body is reported. A 27-year-old man, who had a 2 month history of back pain and paraparesis, was admit-ted to our department in August, 1986. On the first admission, he manifested severe paraparesis and sen-sory disturbance below the Th5 dermatome level. Neuroradiological examination demonstrated an epidu-ral tumor involving the Th4 vertebral body and com-presing the spinal cord. Emergency laminectomy for de-compression and biopsy was performed. The surgical specimen was a giant-cell tumor.
