開頭術後に発生した肺塞栓の2症例

Postoperative Pulmonary Embolism in Neurosurgical Practice: Report of two cases

I．はじめに

　術後合併症としての深部静脈血栓症（deep venousthrombosis，以下DVT）や続発する肺塞栓症（pulmo—nary embolism，以下PE）は，欧米ではよく知られているが，本邦では従来よりまれと考えられ，あまり関心を払われてはいなかった．しかしながら，本邦でも近年増加傾向にあると言われ^3,5,9），特にPEは重篤な経過をたどる場合が少なくなく，早期診断，早期治療が必須であり，重大な術後合併症の1つとしての認識が必要である．最近私達は開頭術後，下肢DVTに起因したと思われるPEの2症例を経験したので，若干の文献的考察を加え報告し，日常臨床においての注意を喚起したい．

Pulmonary embolism following postoperative deep venous thrombosis is a very serious complication with a high mortality rate. Though this disorder has been thought to be rare in Japanese, its occurrence seems to be increasing recently because of changes in eating habits, increase of average age and the frequent prac-tice of venous catheterization.

Two cases of the pulmonary embolism following deep venous thrombosis after surgery are reported, andpossible causes of the deep venous thrombosis are dis-cussed.

Case 1 : A 48 year-old obese female was operated on for a posterior fossa dural arteriovenous malformation.
