環状20番染色体異常に伴うてんかん症候群

環状20番染色体の報告例はこれまでに30例を越すが，てんかんを高率に合併し，その臨床・脳波徴候はユニークである。その特徴を自験12例と文献例をもとに展望した。発作症状の主体は数分から1時間続く非けいれん発作重延状態で，動揺する意識障害を呈する。発作頻度は高く，日単位あるいは週単位で反復する。少なからぬ症例で，局所起源と推定される運動発作ないしけいれん発作を伴う。脳波では両側性高振幅徐波の持続が特徴であり，周波数が変遷し，棘波を伴うことがある。環状染色体の頻度とてんかん症状の重篤さとは相関しない。発作は薬物治療に抵抗することが多い。既知のてんかん遺伝子の欠損は報告されていないが，今後の細胞・分子遺伝学的な病態の解明が待たれる。

There are more than 30 cases with ring chromosome 20 reported in the literature. Epileptic seizures are the most often encountered symptoms of this rare condition. The examination of own 12 cases and cases reported in the literature revealed that the epilepsy associated with ring chromosome 20 provides unique electroclinical features. They are characterized by nonconvulsive status epilepticus with concomitant continuous bilateral high-voltage slow waves with occasional spikes, which usually lasts from several minutes to 1 hour and occurs frequently, often daily.
