　本研究は，双極性うつ病に対するfluvoxamine，milnacipranの有用性についてカルテを用いて後方視的調査を行いparoxetineと比較した。抗うつ薬間，診断亜型間で有用度に差を認めなかった。一方，fluvoxamine，paroxetine，milnacipranに炭酸リチウムを併用した群では，気分安定薬を使用しなかった群と比べて有意に“躁転あるいは不安定化”が少なかった。“やや有用・有用でない”群におけるfluvoxamine，milnacipranの最高用量は，“寛解以上”群と比べて有意に高かった。“躁転あるいは不安定化”と薬剤用量との関連は認められなかった。本研究の結果から，fluvoxamine，milnacipranの双極性うつ病に対する有用度はparoxetineと同等であった。ただし，双極性うつ病に選択的セロトニン再取り込み阻害薬（SSRI）やセロトニン・ノルアドレナリン再取り込み阻害薬（SNRI）を使用する場合には炭酸リチウムの併用が有用と考えられた。

　A naturalistic, retrospective study was conducted to the assess antidepressant effects of fluvoxamine and milnacipran (as compared with paroxetine) as well as their effects on maniac switch or mood lability for treating bipolar depression. No differences were observed in antidepressant effects and maniac switch or mood lability between antidepressants or bipolar subtype diagnosis. Antidepressants, together with lithium carbonate, significantly decreased “maniac switch or mood lability” The highest dose of fluvoxamine and milnacipran in the “mildly effective or no effective” groups were significantly higher than those in the “remission or recovery” groups. These results suggest that the antidepressant effects of fluvoxamine and milnacipran and their effects on maniac switch of mood lability in treatment of bipolar depression are generally equivalent to those of paroxetine. Furthermore, lithium carbonate is beneficial for decreasing maniac switch of mood lability via SSRIs/SNRIs.
