要旨　今回われわれは，気管・気管支軟化症による気道狭窄と悪性腫瘍による気道狭窄の2例に対してPCPSを使用し気道内シリコンステント挿入術を行った．症例1は術前よりPCPSをスタンバイしておらず，術中ステント挿入が困難となったため，緊急にPCPSを確立し，以後安全に挿入できた．症例2は術前からPCPSを確立し安全に完遂することができた．いずれも出血などの合併症を認めなかつた．PCPSのバイパス法にはV-AバイパスとV-Vバイパスの二通りがあり，これらの特徴をよく理解したうえで，バイパス法を選択すべきである．気道内ステント挿入術では，大動脈基部，脳循環により効率良い酸素化が期待され，さらに血管損傷などの合併症も少ないため，V-Vバイパスを選択するほうが適当である．気道内ステント挿入術でのPCPSの使用は今後検討を要するが，常に補助手段の一つとして意識されるべきである．

　We inserted intra trachea stent using PCPS in twopatients whose trachea were stenosed. In the first casethe etiology of the patient's trachea stenosis was tracheomalacia. The operation was rather difficult, so thepatients SaO₂ and systolic blood pressure were down,but her vital signs became stable after we carried out,emergency PCPS and we were able to insert a stentsafely. In the second case, we carrid out PCPS beforeinserting the stent and had no difficulty in inserting thestent. There are two methods of PCPS bypass. One isA-V (arterio-veno) bypass and the other is V-V (Veno-Veno) bypass. We need to fully comprehend the characteristics of these two methods.
