開心術後の稀な合併症である収縮性心膜炎(CP)に対する心膜切除術の1例を報告する．また，3D-CTによるグラフトの同定がこのような症例に有用であることを供覧する．症例は77歳，男性．CABG(4枝)後，経過良好であったが，術後2カ月頃より下肢の浮腫を認め，その後呼吸困難も出現した．CPの診断で初回手術の約8カ月後に心膜切除術を施行した．術前にグラフト造影に加え，3D-CTでグラフトの走行を確認しておいた．手術では3D-CT所見からかなり正確にグラフトが同定できた．その結果，グラフトを損傷することなく広範囲に心膜切除が行えた．

Summary

　Constrictive pericarditis(CP) is uncommon after coronary artery bypass grafting(CABG). We report a case of CP after CABG, followed by radical pericardiectomy without any graft injuries by using three-dimensional computed tomographic angiography(TDCTA). A 77-year-old man with CP after CABG was admitted to our hospital again. Preoperative TDCTA visualized all the grafts precisely, providing their spatial relations to the heart. Based on this anatomic information, radical pericardiectomy was safely corried out while all the grafts were preserved. The postoperative course was uneventful. TDCTA is useful for reducing surgical risk for graft injury in reoperation after CABG.
