要旨●消化管内視鏡診断において，内視鏡所見の特徴を証明する科学的根拠として病理組織像との対比は必須であり，特に拡大内視鏡像と病理組織像との正確な対比が症例検討において重要である．今回，自施設における胃病変に対する拡大内視鏡像と病理組織像の対比方法について概説する．生体内での内視鏡観察方法，内視鏡治療時のマーキング，内視鏡治療直後の検体（ホルマリン半固定後）の割入れと観察方法が手技として必要である．病理検体（ホルマリン固定後）の画像と内視鏡治療直後の検体の画像を用いることで，生体内の拡大内視鏡像と病理組織像を対比することが可能であり，条件さえそろえば内視鏡医のみでも実施可能である．

　In gastrointestinal endoscopic diagnosis, collaboration between endoscopic findings and histopathological images is crucial as a scientific rationale for determining the endoscopic characteristics. In this article, we have outlined the method used to compare the magnifying endoscopic images with pathological findings for diagnosing a gastric lesion. The essential techniques covered in this method include the method of endoscopic observation in vivo, marking during endoscopic treatment, and cutting and observing the specimen soon after endoscopic treatment. The images of the pathological specimen and those of the specimen obtained immediately following endoscopic treatment can be used to compare the magnifying endoscopic image in vivo and the pathological image. This method can be performed only by a qualified endoscopist.
