気管支内に進展する肺カルチノイド腫瘍と重症大動脈弁狭窄症が併存する患者に対し経カテーテル的大動脈弁留置術を先行し，2週後に肺切除術を行った1症例

Resection Performed Two Weeks after TAVI for a Patient with Severe Aortic Stenosis and a Pulmonary Carcinoid Tumor Narrowing the Bronchus

要旨

肺カルチノイド腫瘍の気管支内への進展により高度な気道狭窄を呈する患者が重症大動脈弁狭窄症（aortic stenosis：AS）を合併していたため，経カテーテル的大動脈弁留置術（transcatheter aortic valve implantation：TAVI）を施行し，その2週後に肺切除術を行った症例を経験した。TAVIは待機期間を短縮したい非心臓手術に先行する重症ASの介入法として有用と考えられる。

　We report the case of a 70-year-old male who had both severe aortic stenosis（AS）and severe bronchus stenosis due to the expansion of a pulmonary carcinoid tumor. Before the patient’s tumor resection surgery, he underwent transcatheter aortic valve implantation（TAVI）, and the pulmonary tumor’s resection was performed safely 2 weeks later. TAVI is an useful option as an intervention for severe AS preceding non-cardiac surgeries, especially when a short interval between the treatment of AS and the surgery is desired.
