 End-of-life decision-making and advance care planning(ACP)are becoming increasingly important in Japan’s super-aging society. According to the 2023 White Paper on Ageing Society, nearly 30% of the total Japanese population was aged≧65 years in 2022, and this proportion is expected to reach 38.7% by 2070. ACP is a process in which patients identify their goals and wishes for future treatment and care based on their own values and discuss them with their families and healthcare professionals. Cognitive decline and difficulties in expressing wishes pose significant challenges to the provision of palliative care for older individuals, and healthcare professionals need to provide complex medical information in an easily understood form for both patients and their family members. We discuss the challenges and current status of older individuals in palliative care in Japan and consider measures to improve this care.
