帝王切開麻酔時の硬膜外針による脊髄損傷の1症例

Spinal Cord Injury Caused by an Epidural Needle for Cesarean Section

要旨

　帝王切開における硬膜外麻酔において，Tuohy針の穿刺による脊髄損傷を経験した。患者は急性期に右大腿部の灼熱痛と右下肢の運動障害を訴えたが，徐々に灼熱痛と運動障害は改善した。脊髄幹麻酔に関連した脊髄損傷では機能予後が悪いことが知られているが，本症例では，良好に軽快したため報告する。

　We treated a patient with a spinal cord injury that was due to a Tuohy needle puncture during combined spinal epidural anesthesia for a caesarean section. During the acute phase, the 40-year-old patient complained of burning pain in her right thigh and movement disorders in her right lower limb, but the burning pain and movement disorders gradually improved. Spinal cord injuries associated with neuraxial anesthesia are known to have a poor functional prognosis. This report describes the successful management of a patient with spinal cord injury caused by an epidural needle.
