Epstein-Barr virus（EBV）陽性びまん性大細胞型B細胞リンパ腫（diffuse large B-cell lymphoma：DLBCL）は，60歳以上が約80％と高齢者での頻度が高く，高齢者において予後が不良である。画像所見ではEBV陰性例に対して壊死の頻度が高いと報告されている。今回我々は，CTガイド下生検では診断が困難であり，外科的生検で診断されたEBV陽性DLBCLの1例を経験したので，主として画像所見について文献的考察を加えて報告する。

EBV-positive diffuse large B-cell lymphoma（DLBCL）is more common in the elderly and is associated with a higher incidence of necrosis compared to EBV-negative cases. We report a case in which surgical biopsy of a rapidly enlarging, extensively necrotic abdominal/retroperitoneal lymph node revealed EBV-positive DLBCL. Particular attention should be paid to differentiating this disease from tuberculous lymphadenitis when encountering necrotic lymphadenopathy, especially in the elderly.
