下肢虚血を伴うB型急性大動脈解離に経皮的内膜開窓術を実施した1例

A case of percutaneous intimal fenestration for type B acute aortic dissection with lower limb ischemia

急性大動脈解離の治療はStanford A型であれば外科手術，B型は降圧治療による保存的加療が一般的である。しかし，B型大動脈解離であっても偽腔拡大に伴う分枝閉塞のため，臓器虚血が生じた場合は追加治療が必要となる。原則は手術もしくは大動脈ステントグラフト留置による大動脈修復（central repair）を行うが^1），種々の理由で実施できない症例があり，その場合の治療オプションとして，解離した内膜flapに穴を開け，真腔・偽腔間に人工的に交通を作製する開窓術（fenestration）がある。今回，経皮的内膜開窓術を行った1例を経験したので，報告する。

Percutaneous intimal fenestration is an optional treatment for acute aortic dissection with malperfusion. We report a case of type B acute aortic dissection with lower limb ischemia treated by interventional radiology. RF needle seemed useful because it facilitated the puncture procedure.
