内側型変形性膝関節症の関節温存手術である高位脛骨骨切り術（high tibial osteotomy；HTO）は長年行われている。TomoFixTM plate（DePuy Synthes社）の導入により，初期固定性が向上した開大楔状高位脛骨骨切り術（opening wedge high tibial osteotomy；OWHTO）が主流となり，脛骨内側骨切り部に人工骨を挿入することにより，術後リハビリテーションの早期化が可能となり，広く普及した。本稿では改良型のTomoFixTM Anatomical plateを用いたOWHTOの手術適応，術前計画，手術手技の実際，後療法について述べる。
