Signs and Timing of Sings of Nearing Death of the Elderly Sensed by Nurses in Nursing Homes that Actively Strive to Give the End-of-life Care

抄録

　本研究の目的は看取りを積極的に行っている特別養護老人ホーム（以下，特養）において看護師が高齢者の死期を判断したサインとそのサインを察した時期を明らかにすることである．特養看護師8人に対し，半構造的面接法を用いてインタビューを行い，得られたデータを質的に分析した．その結果，特養看護師が高齢者の死期を判断したサインは19カテゴリーとなり，得られたカテゴリーを高齢者の死に対しての時期で分類すると2段階あることがうかがえた．高齢者の死の約1か月前から発せられる第1段階のサインは【目力のなさ】【皮膚がきれいになってゆくこと】のほか12カテゴリー，約2日前に発せられる第2段階のサインは【呼吸状態の変化】【浮腫出現】のほか3カテゴリーであった．看護師はこれらのサインを見逃がすことなく察知し，家族・医師などと看取りの調整をしていくことが，特養での看取りの質向上のために重要である可能性が示唆された．

　The purpose of this study was to clarify the signs sensed by the nurses from the elderly who are going to die, and the timing of those signs in special nursing homes for the elderly (called ‘nursing home' hereafter) that actively strive to give end-of-life care. Semi-structured interviews were conducted on eight nurses working in nursing homes, and the data were analyzed qualitatively using content analysis. Nineteen signs by which nurses predicted that the elderly death was nearing were categorized. Fourteen categories such as “lack of eye strength”, “losing of persistence to live”, “skin becoming fair” were observed one month before elderly death. Five categories such as “change of respiratory condition”, “appearance of edema” were observed about two days before an elderly death. This study may suggest the importance for nurses to predict the elderly's death by two-phase signs and to provide and coordinate appropriate care for the elderly in order to enhance the end-of-life care in nursing homes.
