外来看護師の在宅療養支援における役割認知尺度の開発

Developing a Role Recognition Scale for Outpatient Nurses in Home Care Support

　目的：外来看護師の在宅療養支援における役割認知尺度を開発し信頼性と妥当性を検討することを目的とした。

　方法：先行文献、半構造化面接を基に58項目の尺度案を作成し、近畿圏内の100床から200床未満の在宅療養支援病院99施設の外来看護師495人を対象に無記名自記式質問紙調査を行い、探索的因子分析、確証的因子分析を行った。

　結果：外来看護師234人（回収率47.2%）から回答を得た。項目分析の結果20項目を探索的因子分析し、3因子15項目が抽出された。因子名を【予測アセスメント役割】、【リレーションシップ役割】、【コミュニケーション役割】と命名した。Cronbach's α係数は尺度全体で0.922と高い信頼性を示した。また、確証的因子分析においても、適合度指標は、GFI = 0.889、AGFI = 0.850、CFI = 0.942、RMSEA = 0.072であり、構成概念妥当性が確認された。

　結論：外来看護師が在宅療養支援の役割を認知するために活用可能な尺度である。

【Abstract】

　Purpose: This study aims to develop a role recognition scale for outpatient nurses in home care support and investigate its reliability and validity.

　Method: A 58-item scale proposal was created based on prior literature and semi-structured interviews. We conducted an anonymous self-administered questionnaire survey on 495 outpatient nurses at 99 home care support hospitals with at least 100 beds, but less than 200 beds, in the Kinki region. We conducted exploratory factor analysis and confirmatory factor analysis on the results.

　Results: Responses were obtained from 234 outpatient nurses (47.2% response rate). As a result of item analysis, 20 items were subjected to exploratory factor analysis, and 15 items across three factors were extracted. The factors were named predictive assessment role, relationship role, and communication role. The Cronbach's α coefficient was 0.922 for the entire scale and showed high reliability. Furthermore, confirmatory factor analysis showed that the fitness indices were GFI = 0.889, AGFI = 0.850, CFI = 0.942, and RMSEA = 0.072, which confirmed construct validity.

　Conclusion: This is a scale that could be used by outpatient nurses to recognise the various roles in home care support.
