右後頭葉皮質下出血後に左四分盲を呈した事例に対して自転車運転の獲得を目指した実践—医師や介護家族との協働

Acquiring the ability to ride a bicycle in a case of left quarter hemianopia after right occipital lobe subcortical hemorrhage: Collaboration with doctors and caregiving families

要旨：今回，右後頭葉皮質下出血により左下1/4半盲を呈した70歳代後半の女性を担当した．この事例に対し，自転車運転の獲得を目的として夫と協働して実車練習をするなどして約4ヵ月取り組んだ．その結果，時間帯や経路の上の制約を認めたものの目標を達成することができた．本事例を通して，半盲患者の自転車運転の再開を目指した作業療法では実車練習を通した安全な方法の確立，医師や介護家族との協働が重要であると示唆された．

　The case is a seventy-year-old woman with left lower quarter hemianopia due to right occipital subcortical hemorrhage. Over a period of 4 months, occupational therapists collaborated with her husband to help the woman acquire bicycle riding skills. As a result, we were able to achieve our goals despite the time and route constraints. We found that occupational therapy in collaboration with doctors and caregivers to resume riding a bicycle for patients with hemianopia through establishment of safe methods through actual vehicle practice is important.
