慢性の関節リウマチを有した脳卒中片麻痺患者に対する作業に焦点を当てた実践—事例報告

Occupation-focused practice for a stroke-induced hemiplegic patient with chronic rheumatoid arthritis: A case report

要旨：本報告の目的は，関節リウマチを既往に持つ脳卒中患者に対して作業に焦点を当てた実践を行うことの有用性を報告することである．本事例は脳卒中による片麻痺と関節リウマチの重複障害を認めていた．そのため，脳卒中および関節リウマチに関する疾患特異的な評価ならびに介入を参照しつつ作業に焦点を当てた実践を行った．その結果，作業遂行能力の向上に加えて上肢機能およびQOLの向上を認めた．さらに，ADLの自立度の向上を認めた．これらの変化は，本事例のような関節リウマチおよび脳卒中などの重複障害を呈した事例に対して作業に焦点を当てた実践を行うことの有用性を示唆する結果であると考えた．

The purpose of this report is to describe the usefulness of an occupation-focused practice for a stroke-induced hemiplegic patient with pre-existing rheumatoid arthritis. We conducted occupation-focused practice while referring to disease-specific assessments and interventions related to stroke and rheumatoid arthritis. Consequently, there was a discernible amelioration in upper limb functionality and quality of life, alongside an enhancement in occupational performance. Furthermore, there was an observable increase in the patient's degree of independence in ADLs. We posit that these improvements underscore the potential usefulness of occupation-focused practice for patients with multiple disabilities, such as rheumatoid arthritis and stroke.
