要旨:通所リハビリテーションを利用する在宅脳卒中患者を対象として,生活活動尺度(以下,LAS)の有用性を検討した.LASの各項目の通過率は8.7〜100.0%の間にあり,個人差を反映しやすいと考えられ,個々の患者について社会的活動の遂行状況を把握しうることが示唆された.さらに,LASに関連するバーセル・インデックス,年齢といった個人要因,家族成員数といった環境要因がそれぞれ影響する活動群が明らかとなった.環境要因が影響する散歩,友人との交際,日用品の買い物といった外出を伴う活動を遂行していない患者では,ひきこもりの可能性が予想された.このような可能性へのスクリーニング検査としてLASの有用性が示唆された.
The purpose of this study was to examine the clinical application of the Life Activity Scale (LAS) with 80 stroke patients living at home, who received day-care services including rehabilitation programs. The LAS consisted of 12 items for social activities, and the patients were asked whether or not each activity was actually performed in their everyday life. The ratio of patients who performed the activities to total patients was calculated for each item. As a result, the ratios across the 12 items ranged from 8.7% to 100.0%. Furthermore, the related factors affecting whether or not social activities of patients were actually performed in everyday life were analyzed. Those factors, including the Barthel Index (BI), the age of the patients and the number of family members, were indicated. The results indicate that patients with more than 80 points on the BI were apt to take a walk, take care of a pet and take a trip in everyday life. Patients under 80 years old were apt to study at home and play indoor games. In addition, patients who lived alone or with one family member had a tendency to take a walk, communicate with friends and go shopping for daily necessities in everyday life. When LAS is applied to stroke patients with different levels of ability and different social backgrounds, the results suggest that it would be able to reflect individual performance profiles in everyday life for social activities among stroke patients. Therefore, LAS is expected to a useful tool for assessment of social activities in stroke patients receiving day-care services.
