著明な前頭葉・記銘障害を呈する症例における俳徊行動に対するアプローチ

Occupational therapy in a treatment of loitering behavior for a patient with frontal lobe symptoms

要旨：重度な前頭葉・記銘障害を呈する症例に対し，目標とする行動を導くための全体的な環境調整を行った．目標を，「妻の買い物に行く間，俳徊行動を抑制する」とした．そのために，「1時間の坐位作業継続を可能にする」という実生活に即した具体的なターゲット行動を設定した．確実にターゲット行動を成し得る条件を見つけるために，様々な条件で確認を重ねた．その結果，“テープレコーダーで3分間隔の具体的指示を与える間，モザイクを行う”という条件を見つけることができた．そして，妻は1時間程度の買い物ができるようになった．

　We adjusted a whole environment to lead the target behavior for a patient with frontal lobe damage and memory disorder. The aim of of the treatment was to control his fugue behavior during a period in which his wife was shopping. For this aim, we set a target behavior which enabled him to work on his sitting position for one hour. Regarding this target behavior, we tried various conditions in a room setting; we monitored the way of direction, the interval of direction, and the activity.

　The resulting condition is as follows : he could make mosaics while he was given the tape recorded directions in three minute intervals, and his wife came to be able to go shopping for about an hour.
