精神障害者をもつ家族の介護負担感に関連する要因―家族支援としての精神科作業療法の在り方の検討─

Factors related to the caregiver burden of families of the mentally ill: Examining the ideal psychiatric occupational therapy method to support families

要旨：精神障害者をもつ家族の介護負担感に関連する要因と介護負担感の軽減を図る手段としての精神科作業療法の在り方を検討することを目的に調査を行った．多数の項目で介護負担感との間に関連を認めたが，その中でも特に多くの正の相関を認めた項目は当事者の精神症状では「感情の平板化」「無関心」「妄想」「遅延」で，自立生活能力では「趣味」「社会的役割」「服薬管理」「受診」であった．これらの結果より，精神科作業療法において陽性症状と同様に陰性症状の改善や当事者が社会との関連の中である程度自立すること，また服薬の必要性を認識し，楽しめる趣味をもて，自分自身の在り方を受容できるよう支援していくことが有効であると考えられた．

　The purpose of this study was to clarify the factors related to the burden of families living with mentally ill family members and to examine the ideal psychiatric occupational therapy method. The following factors correlated with caregiver burden: “Blunted affect”, “Indifference”, “Delusion”, “Delays”, “Hobbies”, “Roles in Society”, “Regulating Medication” and “Consult a Doctor”. Based on these results, our research has shown that psychiatric occupational therapy can relieve positive and negative symptoms. Moreover, we showed that the caregiver burden can be reduced by helping patients both attain self-acceptance and acquire roles in society.
