パーキンソン病のリハビリテーション―最新のエビデンスと姿勢障害へのアプローチ〈総説〉

Rehabilitation of Parkinson's Disease : The Latest Evidence and Approach for Postural Disorders

はじめに

　特発性パーキンソン病（PD）は，振戦，筋強剛，無動，姿勢反射障害を特徴とする進行性の変性疾患で近年増加傾向にあり，人口10万にあたり100～150人にみられ全国で10万人以上の罹患者がいる^1）．

　近年，神経病理学者のBraakら^2）は，Levy小体の出現時期に注目し，延髄の迷走神経背側核をstage1として，中脳と前脳基底部から新皮質にいたる上行性の病変の進行を提唱している．このBraak仮説のよると運動症状が出現する以前から神経変性が進行していることになり，PDの前駆症状として嗅覚障害やREM睡眠行動障害（RBD）が注目されている．また，PDは黒質―線条体系のドーパミン神経系の障害とされてきたが，Alexanderら^3）の大脳皮質―基底核ループモデルが出され，運動系ループ，眼球運動系ループ，前頭前野系ループ，辺縁系ループというパラレルな4つのサブループに分けられている．そして，随意的制御に関する視床を経由した大脳皮質―基底核回路を通じた運動関連領域だけでなく，情動制御に関する辺縁系―基底核回路への投射系や，自動的制御に関する基底核―脳幹回路への投射系の障害が明らかになってきた^4）．

　薬物療法は，初期の運動症状には効果的であるが進行期に入ると認知障害，精神症状，自律神経障害などの非運動症状や多様な姿勢障害や歩行障害がみられ治療に難渋することが多く，リハビリテーション（以下，リハ）の効果が注目されている．また，進行期PDで，薬物療法で運動症状の改善が困難な症例に対して，深部脳刺激法が行われ良好な成績をあげ，同時にリハの介入も行われている^5）．

　これまでのPDリハの報告を踏まえ，特に前屈傾向等の姿勢障害に対するアプローチが初期PDから一貫して必要であることを指摘し，発症初期からリハと薬物療法の併用が極めて重要であることを明らかにする．

Abstract : In 1994, Comella et al published a report of high-level evidence on the effects of physical therapy in patients with Parkinson's disease. Following this, Morris et al suggested a dysfunction of the central rhythm for maintenance function and movement amplitude in PD. They pointed out the improvement in gait velocity and stride, and the expansion of angle of the lower limb joints obtained by using a rehabilitation program. In the advanced stage of PD, an approach catering to the trunk function is important to prevent postural disorders such as camptocormia that can exacerbate gait impairment and ADL disabilities. According to Keus et al, it is necessary to intervene on the basis of rehabilitation strategies according to the stage. Finally, in the rehabilitation strategy of PD it is important to combine medication and exercise of the trunk and lower limbs in order to maintain motor and cognitive function from the start.
