稀なダグラス窩腹膜欠損が原因となった内ヘルニアの腹腔鏡下根治術の1例

A case of internal hernia through a peritoneal defect of the pouch of Douglas successfully repaired by laparoscopic surgery

　腹腔鏡下に根治術を施行し得た，非常に稀なダグラス窩腹膜欠損が原因となった内ヘルニアの1例を経験した．症例は61歳の女性で，腹痛・嘔吐を主訴に入院した．イレウス管留置による減圧を開始し，イレウス管造影では小腸が骨盤内で完全閉塞し，その閉塞部位は仙骨前面に向かっていた．減圧後もイレウスが改善されず，腹腔鏡下手術を施行した．ダグラス窩において小腸が腹膜外に嵌入しており，これを解除すると同部に径1.5cm大の腹膜欠損を認めた．腸壁ヘルニアで腸管の壊死を認めなかったため，腹膜欠損部位を縫合・結紮し閉鎖した．内ヘルニアでも保存的治療で経過観察可能な軽症例では，術前の減圧を兼ねて小腸造影検査などを行い，腹腔鏡下手術で対処することが可能である．

We report a very rare case of internal hernia through a peritoneal defect of the pouch of Douglas, successfully repaired by laparoscopic surgery. A 61-year-old woman was admitted to our hospital with abdominal pain and vomiting. Ileus tube was inserted to the patient, and examination using contrast medium showed a complete ob-struction of the small intestine in the lower part of the pelvis. Lateral view showed that the obstructed site was facing toward the sacral bone. Even after several days of drainage, intestinal obstruction could not be relieved.
