鼻細網肉腫の免疫学的考察—T-cell lymphomaの一症例

IMMUNOLOGIC CONSIDERATION OF RHINOGENOUS RETICULUM CELL SARCOMA: A CASE REPORT OF T-CELL LYMPHOMA

　I．はじめに

　近年，免疫学は多くの分野にわたり，めざましい発展を遂げつつある。リンパ球には由来および機能を異にする二系統のリンパ球，すなわちTリンパ球（thymus derived lymphocyte），Bリンパ球（bone marrow derived lymphocyte）が存在すること，またこれらのリンパ球は適当な刺激に反応して幼弱化（芽球化）すること，さらにはこの二系統のリンパ球がそれぞれの表面構造，表面マーカーのちがいから比較的容易に同定することが可能となつたことなどは，近年における免疫学発展の1つの原動力ともいえる。最近では，Tリンパ球，Bリンパ球の機能および由来を中心に多方面からの検討がなされている。

　一方，従来からリンパ網内系腫瘍の診断，分類は血液学者あるいは病理学者によりその観察力，経験に依存して形態学的になされてきた^1）2）が，近年になつて免疫学の立場からの検討が加えられつつある^3）〜9）。

　With the purpose of clarifying the characterof cells of rhinogenous reticulum cell sarcoma (RR-CS), the tumor cells obtained from a 51 year old male patient were investigated immunologically with 4 membrane markers of B or T lymphocytes. Membrane markers for T lymphocyte consisted of E-rosettes with sheep red cells and indirect immunofluorescence study with specific rabbit antiserum to human T lymphocyte (ALS-T). The B lymphocytes consisted of EAC-rosettes and indirect immunofluorescence study with specific rabitt antiserum to human B lymphocytes (ABS). The diagnosis of T-cell lymphoma was made on the bases of the high percentage of E-rosette forming tumor cells (72%) and the results of immunofluorescence study (positively stained with ALS-T).

　These results support the possibility that some RRCS are lymphatic (presumably T lymphocytic) in origin and not as it is generally assumed to be reticulocytic.
