特異なレ線像を呈した腎腫瘍の1例

HYPERNEPHROMA SHOWING A PECULIAR SHADOW ON THE ROENTGENOGRAM:Report of A Case

　腎腫瘍，殊に副腎腫の手術を困難ならしめ，またその予後を悪くする因子の一つとして腎静脈の腫瘍性血栓症並びにこれと密接な関係にある腎周囲静脈の怒張がある。従つて，若しこれらの面白からざる変化の存在を術前に知る事が出来れば，我々には非常な参考となる訳である。然しながら，この様な点に就ての記載は全く見当らない。我々は副腎腫の1症例に於て，逆行性腎盂レ線像，腹部大動脈レ線像，手術所見並びに別除腎に就ての腎盂内造影剤注入像から考え合わせて，腎静脈腫瘍性血栓症並びに腎周囲静脈の高度の怒張の存在を術前に知り得る可能性を確認し得たので，茲に報告する。

　先ず腎腫瘍，殊に副腎腫の場合の腎静脈腫瘍性血栓症及び腎周囲静脈怒張に関する諸家の報告をまとめて述べてみよう。

The occurances of renal vein involvement and perivenous dilatation are very common in cases of tumor of the renal parencyrna, especially in cases of hypernephroma. Peri-renal venous dilatation in hypernephroma is present in a large majority of cases in which tumor thrombosis of the renal vein is present. These two complications are so serious that the technical difficulties of the operation are increased and that the prognosis is extreme-ly poor. It must be very informative for us that these complications are ascertained be-fore operation.
