自然経過を捕えることができた静脈洞血栓症を伴う硬膜動静脈瘻の1例

The Natural History of a Dural Arteriovenous Fistula Associated with Sinus Thrombosis : a case report

I．はじめに

　硬膜動静脈瘻（以下DAVF）の成因，病態については，今までにも複数の説が唱えられ，各々の説を支持する症例報告も散見されるが，未だ包括的な結論は出ていない．今回われわれは横・S状静脈洞血栓症患者の経過観察中に，静脈洞の再開通と同時にDAVFの新生を認め，さらにその後，静脈洞血栓症の再発と同時にDAVFの増大を偶然にも画像的に確認することができた1例を経験した．今までにDAVFの発生前後の長期自然経過を報告した例は少なく，DAVFの発生機序，及び静脈洞血栓症との因果関係を知る上でも貴重な症例と考え，若干の文献的考察を加え報告する．

The patient, a 52-year-old man, complained of vertigo. CT scan revealed right cerebellar infarction andhe was admitted to our hospital. MRI revealed a fresh thrombus in the right transverse sinus and angiogra-phy revealed the occlusion of the right transverse-sigmoid sinus. He complained of vertigo again and hewas re-admitted to our hospital 4 months after the first admission. Angiography revealed a dural arteriove-nous fistula (DAVF) of the right transverse-sigmoid sinus fed by the right occipital artery and the righttransverse-sigmoid sinus was found to be recanalized.
