Sixty three cases of acute infarction in the territory of vertebrobasilar artery were followed clinically, and development of expanding lesion in posterior fossa was examined by angiography and/or Computed Tomography (CT).

Six cases of them (9.5%) developed massive edema in the posterior fossa at about 24 hours following onset of the ictus, and it caused compression of brain stem and obstructive hydrocephalus. Two of 6 cases were cere-bellar infarction. The other 4 cases were cerebellar in-farction with brain stem infarction; one case with res-tricted brain stem infarction and 3 cases with widespread brain stem infarction.
