深部静脈血栓症の契機となった経尿道的膀胱腫瘍切除術の1例

A case of deep vein thrombosis after transurethral resection of bladder tumor

　症例は87歳，女性。単発の表在性膀胱腫瘍を認め経尿道的膀胱腫瘍切除術（以下，TUR-Bt）を施行したところ，術後2日目に右下肢の腫脹が出現した。CTなどにより深部静脈血栓症の発症を認めた。ヘパリン，ウロキナーゼ，ワルファリンによる治療を開始し，3か月後のCTでは血栓の消失を認めた。TUR-Btは深部静脈血栓症，肺血栓塞栓症の発症の可能性が極めて低い侵襲の小さい手術であるが，高齢，日常生活動作（ADL）の低下などを誘引とし，これらの合併症を引き起こすこともあり，注意が必要である。

　A solitary superficial bladder tumor was found in a 87-year-old woman. Transurethral resection of bladder tumor（TUR-Bt）was done, but right leg pain and swelling appeared two days after the operation. CT examination revealed deep vein thrombosis（DVT）at right common iliac vein level. The combined therapy of heparin, urokinase, and warfarin was started, and the DVT disappeared in the CT three months after the therapy. TUR-Bt is relatively less invasive and the frequency of DVT and pulmonary thromboembolism（PTE）is very low, however these complications may happen especially in elderly people with low ADL.
