汎発性扁平苔癬は比較的まれである1)といわれるが,非定型的な臨床形態を示す扁平苔癬も少なくない2)との報告もある.著者らは最近この異型で汎発性の扁平苔癬を少なからず経験し,臨床診断を迷わされた事があつた.本報告はその代表的6例についてである.
Six cases of atypical generalized lichen planus were presented.
Case 1.-Man, 77, with lichen planus obtusus, had parapsoriasis guttata-like lesions on the trunk and arms and porokeratosis-like maculcs on the back of the feet and lower legs.
Case 2.-Man, 52, with lichen planus annularis, had keratosis follicularis-like lesions with the central clearing.
Case 3.-Woman, 54, with lichen planus pigmentosus, showed reticular, strong pigmentation with spinulous hyperkeratosis mingled with bean-sized depigmented maculcs on the paints, soles and trunk.
Case 4.-Man, 49, with generalized lichen planus, had psoriasis-like lesions consisted of infiltrated erythematous plaque covered with a thick scale among the scattered, dark blueish red nodules on the extremities and face.
Case 5.-Man, 56, with lichen planus erythematosus, had eczema chronicum-like lesion consisted of diffuse, infiltrated, brownish red, irregularly geographical maculcs surrounded with scattered, small erythemas.
Case 6.-Man, 48, with lichen planus erythematosus, had lupus erythematosus-like, sharply defined, in-filtrated erythemas with foliaceous scale scattered on the extremities and back.
In all 6 cases, the lesion had spread over the entire body surfaces within a short period of 1 or 2 months. Histologically, all cases showed typical lichen planus.
Two cases responded to the treatment with arsenic.
Whereas drugs were not confirmed as the causative, the disease was considered as a multi-etiologic disorder in nature.
