The patient, 69 year old man, first noticed onset of a papule on the right calf 12 years ago. It showed a gradual increase in size and finally became a hen's egg-sized tumor with bad-smelling secrction. The tumor was excised and the skin grafting was carried out. Six months later, verrucous nodules appeared in the region where the excision had been performed. The nedulcs gradually increased in number. When he died with cachexia six months after the recurrence, his both legs, back and adbomen were all covered with the nodules. Autopsy revealed that there were metastases in wide-spread areas of the skin and the periaortic lymph nodes. Histopathology of the verrucous nodules: there were metastatic foci of the tumor cells in the epidermis. The primary lesion showed papillary proliferation of tumor cells which were small and rich in chromatin but contained less glycogen. Glandular architecture was scarcely observable and melanocytes were recognized in some areas.

From these findings, it seems likely that the tumor derived from the eccrine acrosyringium. Paget condition noticed in the metastatic foci was considered to be associed with eccrine carcinoma.
