フレイルを合併した難治性めまい患者におけるめまいリハビリテーションと漢方併用療法の効果

Study of combination therapy of vestibular rehabilitation and Ninjin' yoeito for intractable vertigo patients: the effect of combination treatment on frailty

はじめに

　めまい患者は精神的不安を認め^1,2），自宅安静となり，サルコペニアを合併するなど難治化することがある^3）。最近はサルコペニアを中核症状にもつフレイルという概念が注目されている。これは，要介護の手前の虚弱状態を表す症候群で，①体重減少，②疲労感，③歩行速度低下，④活動性低下，⑤筋力低下の5症状（以下，5症状）のうち3症状以上を有する場合にフレイルと診断^4）する。

　そこで今回，慢性難治性めまい（以下，難治性めまい）患者のフレイル合併率が高値であることを明らかにし，難治性めまいの治療にフレイル治療も併せて行う必要性について検討した。最近の研究^5〜7）から，漢方薬の人参養栄湯はフレイルやサルコペニアにも効果が期待されている。これをめまいリハビリテーション（以下，めまいリハ）と併用し，患者を（イ）フレイルと非フレイル，（ロ）65歳未満と65歳以上で分けて比較した後ろ向き研究（横浜市立みなと赤十字病院医療倫理委員会 承認番号2017-33）の結果を報告する。

　We divided 31 inpatients with intractable vertigo into a frailty group and a non-frailty group, and compared the effect of combined treatment with vestibular rehabilitation and Ninjin' yoeito. The results showed improvement in both groups after 6 months of treatment. The frailty group improved to the pre-treatment level of the non-frailty group in several items. It is necessary that the frailty group continues treatment to further improves the symptoms. In addition, when they were divided into age groups（i. e., ＜65 years and ＞65 years）, the latter group was found to have a more benefit from this combination treatment.
