Abstract Purpose:To report an unusual case of penetrating transorbital head injury with exophthalmos.

Case:A 33-year-old male presented with exophthalmos one hour after head injury.

Findings and Clinical Course:Corrected visual acuity was 1.0 right and 0.01 left. Intraocular pressure was 15 mmHg right and 45 mmHg left. The left eye showed proptosis by 7 mm when compared with the right eye. Movement of left eye was limited in all directions. Computed tomography(CT)showed bleeding in the left frontal lobe and subperiosteal orbital hemorrhage. The patient showed hemiplegia and loss of consciousness one hour later and received surgical removal of cerebral and orbital hematoma. After surgery, exophthalmos in the left eye became severer and was treated by pulse therapy with methylprednisolone. The cornea was protected by a plastic sheet. Exophthalmos disappeared with visual acuity of 1.2 in the left eye 11 months after injury.

Conclusion:The present case illustrates that CT may be useful in the diagnosis of head injury with orbital involvement. Orbital inflammation appears to have caused additional exophthalmos following surgical removal of orbital hematoma.
