葡萄膜転移癌の保存的治療

Conservative treatment of metastatic cancer to the uvea

　3例の葡萄膜転移癌を報告した。症例は，肺癌の両眼脈絡膜転移，乳癌の左眼脈絡膜転移，肺癌の両眼脈絡膜，右眼虹彩転移であった。いずれも保存的治療として放射線照射あるいは光凝固を施行し，視力を保存または改善することができた。3例ともに血漿Carcinoembryonic antigen （CEA）が高値を示した。死亡2例中1例については病理組織検索を行い，肺原発巣に一致するadenocarcinomaを脈絡膜，虹彩に認めた。

Three cases of metastatic carcinoma to the uvea were treated by concervative means. The first case, a 68-year-old male developed choroidal me-tastasis in both eyes originating from adenocarci-noma of the lung. A small lesion in the right eye was photocoagulated by xenon. The left eye, in which the retina was totally detached, was irradiated with Linac. The tumor recurred 9 months later in the right eye and was treated by additional irradiation. The visual acuity in the right eye was maintained at the same level through-out the course of treatment.
