要約 目的:緑内障性視野障害と乳頭周囲の網膜神経線維層厚との関係の報告。対象と方法:広義の開放隅角緑内障40例50眼を対象とした。年齢は48~83歳(平均67歳)である。視野はHumphrey自動視野計のglaucoma hemifield testで測定し,スペクトラルドメイン光干渉断層計で乳頭周囲の網膜神経線維層厚を測定した。結果:全症例で,上耳側・下耳側の網膜神経線維層厚はセクター2・3の視野と相関があり,耳側の網膜神経線維層厚はセクター5と相関を示した。早期の25眼では,上耳側・下耳側の網膜神経線維層厚はセクター3のみと相関を示した。結論:乳頭上耳側と下耳側の網膜神経線維層厚は,緑内障の早期診断の指標となる可能性がある。
Abstract. Purpose:To report the correlation between glaucomatous visual field defect and thickness of parapapillary retinal nerve fiber layer(RNFL)measured by spectral domain optical coherence tomography. Cases and Method:This study was made on 50 eyes of 40 patients with open-angle glaucoma in the wider sense. The age ranged from 48 to 83 years,average 67 years. Visual field was measured by glaucoma hemifield test of Humphrey perimeter. Thickness of parapapillary RNFL was measured by spectral domain optical coherence tomography. Results:Retinal sensitivity in sector 2 and 3 was significantly correlated with thickness of RNFL in the superior and inferior temporal sector respectively. Retinal sensitivity in sector 5 was significantly correlated with thickness of RNFL in the temporal sector. In 25 eyes with early-stage glaucoma,thickness of RNFL in superior and inferior temporal sector showed significant correlation with retinal sensitivity in sector 3 only. Conclusion:Diagnosis of glaucoma may be facilitated by measurement of thickness of RNFL in superior and inferior temporal sectors.
