Abstract. Purpose:To report a case of cytomegalovirus(CMV)retinitis who developed retinal detachment following immune recovery uveitis(IRU). Case:A 49-year-old male presented with CMV since 6 months in the right and one month in the left eye. He had history of AIDS,toxoplasmosis,chronic hepatitis B,and Kaposi sarcoma. He had been receiving ganciclovir and highly active antiretroviral therapy(HAART). Clinical Course:His corrected visual acuity was 1.2 right and 1.5 left. CMV retinitis recurred in both eyes after withdrawal of ganciclovir and HAART due to impaired liver function. IRU developed in the left eye 4 months later,followed by retinal detachment due to vitreal traction and retinal break that formed along the borderline posterior to the area of CMV retinitis. The retina became reattached after multiple surgeries with final visual acuity of 0.5. Retinal area affected by CMV retinitis remained attached throughout. Conclusion:The retina in the area of CMV retinitis remained attached presumably due to adhesion between the sensory retina and retinal pigment epithelium even in the presence of IRU.
