変形性膝関節症に対する採型した外側楔状足底板は1年経過すると効果が減弱する

No Symptomatic Benefits Custom-made Lateral-wedged Insoles for Patients with Osteoarthritis of the Knee at the One-year Assessment

　背景：採型足底板は変形性膝関節症(膝OA)患者の足型に合っているが，毎月交換するためには高価である．

　対象と方法：この研究では採型足底板と1カ月ごとに交換する既製足底板の1年後の治療効果を比較した．

　結果：既製足底板を毎月交換した患者(20例)では治療前に比し，VASが有意に改善した(P＜0.0001)が，採型足底板を装着した患者(n＝18)では有意な変化はなかった(P＝0.16)．

　まとめ：既製足底板などを用いた膝OAに対する保存療法によって低価格で効果的な治療を行えるようになれば，患者にも高齢化社会の医療経済にも役立つであろう．

　Background：Custom-made lateral-wedge insoles fit the foot patterns of each patient with osteoarthritis (OA) of the knee, but they are costly to be replace every month.

　Method：A recent study compared the symptomatic benefit of custom-made insoles and ready-made insoles that had been replaced every month.

　Outcome：At the end of one year the patients who wore the ready-made insoles that had been replaced every month (n＝20) had significantly improved visual analogue scale (VAS) scores (P＜0.0001) that at the baseline assessment.　Significant difference is not found in the group that wore the custom made insole (n＝18) (P＝0.16).

　Conclusion：Conservative therapy for OA of the knee, such as wearing a ready-made insole, can provide low-cost and effective treatment, it will be very useful for patients and the healthcare economy in our aging society.
