手術時間の延長は人工膝関節置換術(TKA)後感染の危険因子と考えられるが,これを証明した研究は少ない.同一プロトコルで連続して行った初回TKA541関節を対象とし,これらを感染群と非感染群に分け調査した.両群の年齢,性別,身長,体重,BMI,診断,術前JOAスコアに差はなかった.手術時間は感染群126分,非感染群95分であり,感染群で有意に長かった(p=0.015,Mann-Whitney U-test).手術時間の延長はTKA後の感染率に関与しうる.
Whether there is a correlation between total knee arthroplasty (TKA) operative time and postoperative infection is a matter of controversy. We conducted a retrospective study of patients who underwent primary TKA according to the identical protocol and compared operative time in the infected cases and non-infected cases. There were no significant differences in age, sex, height, weight, body mass index, or preoperative JOA scores between the two groups. Operative time was longer in the infected cases (126 min vs. 95 min;p=0.015, Mann-Whitney U-test). Our findings support the view that long operating times are associated with a higher rate of postoperative surgical site infection after TKA.
