閉所恐怖症状が上肢ギプス固定に及ぼす影響について

Arm Cast Intolerance of Claustrophobic Patients

　上肢手術の術前後に行ったギプス，ギプスシーネまたはアルフェンスシーネ固定に対して不快感や閉塞感を強く訴え閉所恐怖症状を呈した6例を経験した．閉所恐怖症状を確認した後，固定との関連性を説明し，ギプスシーネ，アルフェンスシーネ固定用の包帯または手関節装具のすき間から皮膚をみせることで閉じ込められた空間ではないことを納得させた．不安感を和らげることができ，固定期間を順調に経過した．ギプス固定に不快感を過剰に示したり，自己抜去するような通常ではない反応を示す症例では，閉所恐怖症状の有無を確認することが重要と考える．

　We report six cases in which a claustrophobic patient complained of severe anxiety in regard to an arm cast immediately after it was applied.　We discussed the possibility of cast discomfort related to their claustrophobia with the patients.　Splints were applied instead of the casts, and the patients were able to see part of their forearm through the splints.　This modification was effective in reducing both their anxiety and feeling of being trapped, and in obtaining their cooperation in regard to treatment.
