　後頭蓋窩腫瘍により有痛性斜頚を呈した7歳男児の症例を経験したので報告する．初診時，有痛性斜頚と3D-CTにより環軸椎回旋位固定の診断を下した．持続介達牽引治療により斜頚はいったん改善したが，初診から4週間後に反対側の斜頚の再発を呈した．再発時には失調性歩行も呈しており，頚椎MRIで後頭蓋窩腫瘍を指摘され脳神経外科に紹介された．後天性斜頚の診療にあたっては腫瘍性疾患や感染性疾患の可能性を常に考慮することが必要である．

　A 7-year-old boy presented with painful left torticollis and restriction of neck movements.　There was no history of trauma or of any systemic disorder that is associated with atlanto-axial rotatory fixation.　The torticollis and neck pain resolved after two weeks' of traction with a halter.　However, right torticollis and an ataxic gait developed 10 days after discharge, and MRI revealed a posterior fossa tumor as the cause of the torticollis.　A posterior fossa tumor must be included in the differential diagnosis in cases of acquired torticollis.
