鎖骨にtraction spurと角状変形，胸鎖関節亜脱臼を来した2歳の筋性斜頚の1例

Congenital Muscular Torticollis with Traction Spur, Angular Deformity of the Clavicle and Subluxation of the Sternoclavicular Joint. Case Report

　胸鎖乳突筋の鎖骨枝の牽引力により鎖骨に骨棘と角状変形を来し，胸鎖関節亜脱臼を生じた極めて稀な，2歳の先天性筋性斜頚の1例を経験した．胸鎖乳突筋鎖骨枝の緊張が強く，全身性関節弛緩性を伴っていた．3歳時，左胸鎖乳突筋下端部部分切除術を施行した．骨棘切除は行わなかった．3歳4カ月時，胸鎖関節部の動揺性は消失し，X線像でも骨棘は縮小し，鎖骨角状変形は自然矯正され，胸鎖関節は整復位となった．筋性斜頚において，鎖骨の形状，肩関節機能にも注意する必要がある．

　We report a rare case of congenital muscular torticollis associated with a traction spur, angular deformity of the clavicle, and subluxation of the sternoclavicular joint.　The patient had generalized joint laxity, so we speculated that the traction on the clavicle caused the subluxation of the sternoclavicular joint.　We partially resected the sternocleidomastoid muscle, and reduction of the spur, angular deformity, and the sternoclavicular joint were seen on an X-ray follow-up examination four months after surgery.
